Prompt Services for Urgent Needs
If your loved has passed at home and is under hospice care, please call the hospice provider. When you are ready for us to come take your loved one into our care, hospice will call us.
If your loved one passed at home and is not under hospice care. You need to call the police to come to the home. They will give us a call when they arrive.
In a Facility (Nursing home, Rehabiliation Center, assisted living):
The facility or hospice provider will give us a call when you are ready for us to take your loved one into our care.
Hospital
Please give us a call to let us know your loved one has passed. We will reach out to the hospital to set up a time to take your loved one into our care.
We will follow up with you if you have not made prior arrangements with us to send over our forms via DocuSign or set up a time to do the forms in person. The forms will need to be filled out by the legal next of kin.
Next of Kin
Is there a Power of Attorney for Healthcare? Yes. We will need a copy provided to us. We do not need anyone else to sign.
If there is no Power of attorney for Healthcare the order of Next of Kin is as follows:
Next closest relative (grandparents, aunt or uncle not through marriage.
